South West Birmingham Primary Care Network

As NHS reforms work to improve the integration between health and social care providers we are now working more closely as part of the South West Birmingham PCN (primary care network).  This network sees us working with some of our most local GP colleagues to deliver services collectively to a population of almost 50,000 people.  It will also see an enhanced number and variety of services available within the community at primary care (general practice) level.  One of our greatest successes to date has been the collective delivery of the Covid vaccination programme.

Over the coming months and years we will become an increasingly diverse, multi-disciplinary team including social prescribers, pharmacists, paramedics, physiotherapists, mental health nurses and many more besides.  We will also progress to work more closely with our colleagues in social care and community healthcare.

As well as Woodland Road Surgery the SWB Primary Care Network (PCN) includes five other local practices covering the areas of Northfield, West Heath, Cofton, Longbridge, Shenley and Kings Norton.
